Clearance Packet

It is a PA State Law that all adult volunteers (age 18+) in a school setting must obtain their clearances, and must renew, at least, every 5 years.  If your clearances are not on file, you WILL NOT be able to volunteer for school activities including, but not limited to Class Parties, Field Trips, Library Committee, Book Fair, and other Special Events.

Required Clearance:

• Act 34 Criminal Record - Every 5 years

• Act 151 PA Child Abuse History - Every 5 years

• Act 114 Federal FBI Fingerprinting - Every 5 years

  • Click this link if you HAVE lived in PA for the entire past 10 years 

    • Fingerprinting Waiver - Every 5 years - Use this form in lieu of Fingerprinting

Once clearances are completed please submit them electronically to either the APC or AES school offices.  Also, retain a copy of your clearances for your records.
